When a participant experiences a breakdown in informal supports, hospital discharge without safe housing, or an unsafe living situation, rapid and structured response is critical. Holistic Integrated Care Services provides short-notice Emergency Accommodation with embedded clinical oversight. From day one, our nursing team completes assessment and triage to ensure medications, medical equipment, behavioural risks and environmental safety are properly managed. This is not simply temporary shelter. It is stabilisation, safety and coordinated planning for the next step — whether that is SDA, SIL, ILO or another long-term housing solution.

24–48 Hour Intake & Assessment

We coordinate with hospitals, Support Coordinators and families to secure stable long term housing.
